A recent study led by SickKids Hospital and the Montreal Children’s Hospital in Canada has found that children with peanut allergies may not require high doses of oral immunotherapy to develop protection.

Published in the Journal of Allergy and Clinical Immunology, the research suggests that low-dose peanut exposure can help reduce the risk of severe allergic reactions from accidental exposure, while minimizing side effects associated with standard treatment methods.

Peanut Allergy in Children
Peanut allergy affects approximately 2% of children and adults in Canada and is a growing cause of hospital admissions. Oral peanut immunotherapy aims to train the immune system to tolerate higher amounts of peanuts, reducing the severity of reactions upon accidental exposure.

Study Details
The study involved 51 children with peanut allergies, who were randomly assigned to one of three groups:

  • Low-dose oral peanut immunotherapy

  • Standard-dose oral peanut immunotherapy

  • Peanut avoidance without immunotherapy

Both immunotherapy groups showed similar improvements in reducing allergic reactions, indicating that small peanut doses are more effective than avoidance in training the immune system.

Dr. Julia Upton, the study’s lead author, stated that the findings were encouraging, highlighting that lower doses delivered positive outcomes with fewer adverse reactions, helping children stay on treatment.

Children receiving a 30 mg maintenance dose experienced fewer side effects compared to those on a 300 mg dose, making the treatment more tolerable—especially for children sensitive to taste.

Future Implications
Researchers believe these findings pave the way for more flexible and personalized treatment protocols, allowing families to choose safer, more manageable options, and ultimately expanding access to effective peanut allergy treatments for children.
