Digestive causes are common, but not limited to the colon. Gas buildup can cause sudden cramps and bloating, especially after heavy meals, often relieved after passing gas or bowel movements. IBS causes recurring lower abdominal pain linked to stress and irregular eating habits.
More serious digestive causes include gallstones, which trigger sharp pain in the upper right abdomen, sometimes radiating to the shoulder or back, especially after fatty foods. Appendicitis is another emergency condition, starting with mild pain near the navel before shifting sharply to the lower right abdomen, accompanied by fever, nausea, and loss of appetite.
Urinary causes should not be overlooked. The right kidney may be responsible, especially in cases of kidney stones, which cause severe pain radiating to the back or groin, often with blood in urine or burning during urination. Kidney infections can cause fever, fatigue, and nausea, requiring urgent antibiotic treatment.
In women, right abdominal pain may relate to ovarian cysts, hormonal disorders, or fallopian tube infections. In rare but critical cases, it could indicate an ectopic pregnancy, particularly if accompanied by bleeding or dizziness.
Sometimes, the pain is purely muscular—caused by strain, poor posture, or nerve irritation—and usually worsens with movement and improves with rest.

If the pain is sudden, worsening, or associated with fever, persistent vomiting, cold sweats, dizziness, or breathing difficulty, urgent medical evaluation is essential.
