In a medical milestone that reshapes what the world knows about HIV, researchers confirmed the full recovery of a German man—known as B2—after six years without detectable virus levels, marking him as the seventh patient ever to achieve complete HIV remission.

His journey began in 2015 when he underwent a stem cell transplant to treat aggressive leukemia. The donor carried a single copy of the CCR5 Δ32 mutation—previously thought insufficient to provide strong or lasting resistance against HIV. Yet the outcome defied expectations: in 2018, B2 discontinued antiretroviral therapy, and the virus has not reappeared since.

This case shines new light on how stem cell transplants can dismantle HIV’s persistent reservoirs—those hidden immune cells where the virus lies dormant for years. High-intensity chemotherapy wipes out most of the infected immune system, while donor stem cells rebuild a new one capable of tracking and eliminating lingering viral traces through a phenomenon known as graft-versus-reservoir response.

While previous successful cures involved donors with two copies of the rare CCR5 Δ32 mutation—making their immune cells naturally resistant to HIV—B2’s recovery, along with a similar case known as the “Geneva patient,” suggests that complete resistance may not be the only path to a cure.

Although stem cell transplantation is far too risky to be a standard HIV treatment, the insights gained from these cases inspire new therapeutic strategies. Researchers hope to replicate the key mechanisms—reservoir reduction and partial CCR5 protection—through gene editing and advanced drug therapies.
